| CAMP REGISTRATION |
Thank you for enrolling in California Field Hockey Camp at San Jose State.
Camp dates are July 28-Aug. 1, 2008. Check-in time is Monday at 8:30 at the fields on the San Jose State campus at Tenth and Alma streets. Be dressed to play and bring your stick, water, shin guards, mouthguard, sunscreen, etc. Sessions on Monday, Wednesday and Friday are from 9:00am -1:00pm. Tuesday and Thursday will run from 9:00am -12:00pm and 1:00 -3:00pm. You should bring food and drink for your lunch break on Tuesday and Thursday.
I hope to have a vendor on site, but players should bring their own stick and goalies their own pads, gloves, helmet and stick. Balls will be provided at camp. See you in soon!

Individual Registration
$205.00 per player. Fee and form due June 30, 2008.
Individual Camp Registration Form
Team Registration
$180.00 per player (Team = 10 player minimum). Team registrations are made through your coach or designated team representative. Teams must have a minimum of 10 players to qualify for the team rate and all team registrations MUST be received at the same time. No team rate unless postmarked by June 15, 2008. Team space assignments will be made on a first come basis.

Each participant registered will receive a reversible camp jersey. Please check size desired on registration form.

Days/Times:
July 28-August 1, 2008
Monday, Wednesday and Friday: 9:00 am - 1:00 pm
Tuesdays and Thursdays: 9:00 am - 12:00 noon and 1:00 - 3:00 pm.
Afternoon sessions are devoted to coached games and team play.
Players are stongly encouraged to bring a lunch on Tuesdays and Thursdays.

Housing
Housing can be obtained at local hotels or through the San Jose State housing office by contacting (408) 924-6196 for dorm rooms. A responsible chaperone must accompany any overnight campers or teams. Please note that the dorms are not located close to the fields and will require transportation.
Equipment
All players should bring their own sticks. Leg guards and mouth guards are strongly recommended. Loaner sticks are NOT available.
Important
Please complete the registration form, medical form and check the appropriate boxes. Registered campers will receive a confirmation notice. The medical release must be completed and returned before participation is permitted.

Refunds
Refunds are applicable to individual registration. If withdrawal is necessary, request must be received no later than July 1, 2008.
Refunds will be less $25.00 of total fee. A $25.00 fee will be charged for returned checks. Substitutions are accepted on team rates; however, refunds will not be made. |
