To Team Coaches/Representatives:
Here are the registration materials for the California Field Hockey Camp at San Jose. The team rate of $195.00 includes the jersey, seven sessions of instruction and daily snack. To qualify for the team rate, you must have a minimum of 10 players AND all medical waivers , checks and team roster, to be received at the same time. Please see details below. Registrations must be postmarked on or before June 19, 2010. Groups of less than 10 players or any individual registration will pay the regular $225.00 fee per camper.
Please make arrangements with your players to have the proper shoes, shinguards, mouthguards and sticks and goalkeeping gear.
PROCEDURES FOR YOUR TEAM REGISTRATION
1. Each player needs to complete the the authorization to consent to treatment of a minor waiver. Please check the information yourself to insure it is filled out properly. The insurance information must be complete and the waiver signed before a player can do their own online registration. Please make extra copies as needed.
2. Fill out the online TEAM registration form with your player’s complete names AND THEIR email addresses. Please indicate which players are beginners. Also on the form, indicate if you wish for your players to stay together as a team throughout camp or be divided according to ability. The maximum roster for one team is 16.
3. Send a copy of your team registration form (which will print out upon submission) to us with your total payment AND completed waiver forms. We will then reserve a place for your team. At least 10 registrations must be sent with payment to qualify for the team rate. Groups of less than 10 will not be accepted unless the regular fee of $220.00 per camper is included.
4. Upon receiving your team payment and waivers, we contact you to direct your players to fill out their own individual registration forms online. If a player does NOT have internet access, please let us know in the comments on the registration form.
5. The camp does not refund team registrations. Substitutions are welcome. Your player must find a substitute, not California Field Hockey Camp. Please inform your athletes of this information. All substitutes must submit a registration form and completed insurance form/waiver.
6. Please provide a live-in chaperone if your team plans to stay in the dormitory.
